New patient consultation: $500
Follow-up visit: $300
The fee is the same whether we meet by secure video or, on request, in person.
Payment in full is collected when you book.
Laboratory work and pharmacy costs are billed separately by those providers.
This practice does not bill insurance. You may request a receipt to submit for out-of-network reimbursement, though reimbursement is not guaranteed.
If you do not use insurance, you have the right to receive a Good Faith Estimate of expected charges. The fees above are the full fees for each visit, and you may ask for a written estimate at any time.
Cancellations and rescheduling
Your appointment time is reserved just for you. Please give at least 24 hours' notice to cancel or reschedule, and your visit fee can be applied to a new time. For cancellations with less than 24 hours' notice, no money is returned, and you will be offered the option to reschedule. If I need to cancel or reschedule, you will receive a full refund or your choice of a new time.
