This website is operated by Meridian Performance & Longevity PLLC, doing business as Dr. Juan Luis Castro Cordoba, MD, MPH | Psychiatry & Hormone Care (the "Practice"). This policy explains what information this website collects and how it is used. It is separate from the Notice of Privacy Practices, which explains how your health information is protected once you are a patient.
Please do not send health information through this website
This website is not for sending symptoms, diagnoses, medications or other medical details. Please do not use contact forms, regular email or text messages for health information. Health information is collected only through the Practice's secure patient portal.
Information we collect
Information you give us. If you call, email or write to the Practice, we receive your name, contact details and the contents of your message.
Information collected automatically. Our website host, Squarespace, collects basic visit data such as pages viewed, browser and device type, approximate location from your IP address, and referring page. We use this data in summary form to understand how the site is used and to keep it secure.
Booking and payment. When you book, you leave this website and use a secure third-party scheduling, patient portal and payment system. That system collects the information you enter, including payment details, under its own privacy terms. The Practice's website does not store your payment card number.
