Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Effective date: October 11, 2026

This notice applies to Meridian Performance & Longevity PLLC, doing business as Dr. Juan Luis Castro Cordoba, MD, MPH | Psychiatry & Hormone Care (the "Practice"). It describes how the Practice uses and shares your protected health information and your rights under the federal HIPAA Privacy Rule and Florida law. The Practice is required by law to keep your health information private, to give you this notice, and to follow its terms.

How we may use and share your information

Treatment. We may use and share your information to provide, coordinate and manage your care, for example with your pharmacy, your laboratory, or another clinician treating you.

Payment. Because this is a private-pay practice, we do not bill insurance. We may use your information to process your payment, issue receipts and, if you ask, give you a receipt to submit for reimbursement.

Health care operations. We may use your information to run the Practice, such as quality review, training, scheduling, record keeping and use of secure technology vendors who are bound by written agreements to protect your information.

Other uses and disclosures allowed or required by law. These include: when required by law; public health activities; reporting suspected abuse, neglect or domestic violence; health oversight by government agencies; court orders and legal proceedings; law enforcement as the law allows; to prevent or lessen a serious and imminent threat to the health or safety of a person or the public; to coroners and medical examiners; and for workers' compensation as the law requires.

People involved in your care. With your agreement, or when you do not object, we may share limited information with a family member or other person you name who is involved in your care. For a minor patient, we may share information with the parent or legal guardian as permitted by law.

Uses and disclosures that need your written authorization

  • Most uses and disclosures of psychotherapy notes.

  • Uses and disclosures of your information for marketing.

  • Any sale of your information. The Practice does not sell your information.

  • Any other use or disclosure not described in this notice.

You may cancel an authorization in writing at any time. This will not undo what was already done while the authorization was in effect.

Substance use disorder records

If the Practice ever receives or creates records that are covered by the federal rule on substance use disorder treatment records (42 CFR Part 2), those records get additional protection. They will not be used or shared in legal proceedings against you without your written consent or a court order that meets the rule's requirements.

Your rights

  • See and get a copy of your health records, in paper or electronic form, usually within 30 days of a written request. A reasonable, cost-based fee may apply.

  • Ask for a correction if you think information in your record is wrong or incomplete.

  • Ask for confidential communications, such as being contacted at a different phone number or by a certain method. We will agree to reasonable requests.

  • Ask for limits on how we use or share your information. We are not required to agree, with one exception: if you pay for a service in full out of pocket and ask us not to share information about that service with your health plan, we will agree, unless the law requires us to share it.

  • Get a list of certain disclosures we made of your information in the six years before your request.

  • Get a paper copy of this notice, even if you agreed to receive it electronically.

  • Choose someone to act for you, such as a legal guardian or a person with medical power of attorney. We will confirm that the person has this authority.

Our responsibilities

  • We keep your health information private and secure as the law requires.

  • We will tell you promptly if a breach occurs that may have put your information at risk.

  • We follow the duties and privacy practices described in this notice and give you a copy of it.

  • We will not use or share your information other than as described here unless you give us permission in writing.

Electronic communications and telehealth

Visits take place on secure, HIPAA-compliant technology. Health information is collected through the Practice's secure patient portal. Regular email and text messages are not secure, so please do not send health details that way.

Changes to this notice

We may change this notice and the new terms will apply to all the information we hold. The current notice is always posted on this website and available in the patient portal and on request.
