Telehealth Consent
Last updated: October 11, 2026
Please read this consent carefully. By booking a visit and signing this consent in the patient portal, you agree to receive care from Dr. Juan Luis Castro Cordoba, MD, MPH, through Meridian Performance & Longevity PLLC (the "Practice"), by telehealth. If you are the parent or legal guardian of a minor patient, "you" in this consent refers to you and to the child.
1. What telehealth is
Telehealth means receiving health care by secure video, or by phone when video is not possible, instead of being in the same room as your doctor. It includes evaluation, diagnosis, treatment planning, medication management and follow-up.
2. Benefits and limits
Telehealth can be as effective as in-person care for many conditions, and it is convenient and private. It also has limits. Your doctor cannot do a physical examination or draw blood by video. Technology can fail, and the connection or audio may be interrupted. If your doctor decides that telehealth is not appropriate for you, you will be told, and you may be asked to see another provider or to come in person.
3. Where you must be
You must be physically located in the State of Florida during every visit. At the start of each visit you will be asked to confirm your location, and you agree to give your doctor a street address and a phone number where you can be reached, so that help can be sent if there is an emergency. You agree to tell your doctor at once if you are not in Florida, and the visit may have to be rescheduled.
4. Identity and privacy
Your doctor will confirm your identity at the start of care. Please take part from a private space, with no one else listening unless you have agreed to it. Visits use secure, HIPAA-compliant technology. You agree not to record any visit, and the Practice will not record visits, unless you and your doctor agree in writing first. Please do not share the visit link.
5. Children and teens
A parent or legal guardian must give consent for a patient under 18 and must be available at the start of every visit. Children and teens are seen by video from a private place with an adult available. The Practice treats patients ages 5 to 17.
6. Emergencies and crises
This Practice does not provide emergency services and is not monitored around the clock. If you are in a crisis or emergency, call 911 or go to the nearest emergency room. For the 988 Suicide & Crisis Lifeline, call or text 988.
7. Contact between visits
You may call (954) 228-4560 and leave a message. The Practice aims to return calls within 24 hours. Do not use voicemail, text or regular email for urgent matters or to send health details.
8. Medication and prescribing
Decisions about prescribing are made case by case after a full evaluation and follow state and federal law. Some treatments, including some medications, may require an in-person visit, an exam, or laboratory tests. Some medications cannot be prescribed by telehealth. Your doctor may decline to prescribe a medication that is not safe or appropriate for you.
9. If technology fails
If the video connection fails, your doctor will try to reach you by phone at the number you gave. If the visit cannot continue, it will be rescheduled. Please keep your phone nearby during every visit.
10. Your records and confidentiality
Your records are kept as your Practice's medical records and are protected by HIPAA and Florida law. Information is shared only as permitted by law and described in the Notice of Privacy Practices. There are legal limits to confidentiality, including when someone is at serious risk of harm and when abuse or neglect must be reported.
11. Fees and payment
This is a private-pay practice and does not bill insurance. Fees are listed on the Fees page and are due in full when you book. Cancellation terms are on the Fees page and apply to your booking.
12. Your choices
Your consent is voluntary. You may stop telehealth and ask for in-person care or a referral at any time, and doing so will not affect your right to receive care elsewhere. You may withdraw this consent in writing by contacting the Practice.
13. Acknowledgment
By signing in the patient portal, you confirm that you have read and understood this consent, that you have had the chance to ask questions, and that you agree to receive care by telehealth.
